The Forensic Outreach and Liaison Service is a service for Kent, providing a safe, effective and timely pathway from medium/low secure care into the community, for people requiring ongoing specialist forensic mental health care following a secure inpatient admission. The service will provide ongoing care and support for those individuals requiring specialist intervention to address risks and psychological needs and works to support social care needs, which if these failed could potentially lead to relapse and reoffending. The service is within the South East, Adult Secure Services, New Care Model and will work in parity and close partnership with Surrey and Sussex Forensic Services. Main duties of the job The post holder will work within the community forensic and liaison service, based at the Trevor Gibbens Unit with close support from the Mental Health Practitioners and Senior STR worker. This role will be part of a multi-disciplinary team, with a commitment to delivering recovery focused, patient centred care to people with mental disorder/illness, who have offended or are at risk of offending. The service is provided in the community, as well as working closely with inpatient teams in secure hospitals to ensure the safe and efficient discharge of patients to the community.
